If you are interested in joining TSA, you can download the necessary membership documents here. Every new member is required to complete and sign a (1) Membership Application, (2) a Skills Inventory, and (3) a Waiver and Indemnity form. In addition, a parent or guardian of an applicant who is under the age of 18 must have the Application and the Waiver and Indemnity Forms signed by a Parent or Guardian. The signature of the Parent or Guardian on the Waiver and Indemnity Form must be notarized. If an applicant is married, the spouse must sign the Waiver and Indemnity Form as well as the applicant.
